Read the individual assay
Quest and Labcorp publish different serum B12 intervals and different serum folate thresholds. Serum and RBC folate also represent different measurements. Copying a number between assays or specimen types can obscure the question a particular result was designed to address. [1] [2]
Understand what additional testing may clarify
NIH describes methylmalonic acid as a useful additional marker when serum B12 is uncertain, while also noting interpretation issues such as kidney function. This is an example of contextual assessment rather than a rule that every result in the lower part of an interval represents the same condition. [1]
Keep recent intake in the record
Serum folate is influenced by recent intake. Supplements and other clinical factors also matter when interpreting a B12 result. Record these details alongside the laboratory measurement so that a change is not interpreted without its collection and clinical context. [1] [2]
Separate nutrient biology from broad classifications
Established roles in one-carbon metabolism do not validate every commercial methylation panel or historical psychiatric biotype label. A proposed optimal interval needs its own source and evidence. This guide does not assign a universal B12 or folate target to Pfeiffer, Walsh or Hoffer without an identifiable primary basis. [1] [2]
